Day center attendance is based on individual need or choice and can range from once to several days a week. Day Health Center attendance is not mandatory for the SeniorLink Home-Based Program. SeniorLink provides transportation to and from home and the center as well as to appointments with specialists and other activities.

Home-Based Option
SeniorLink's Home-Based Team offers SeniorLink services to appropriate participants without requiring attendance at one of the SeniorLink day health centers. The team includes a physician, nurse, social worker, dietitian, and physical, occupational and recreational therapists. The team considers many factors when assessing potential participants for the Home-Based program, including mental status, psychosocial issues, medical situation, and the needs and interests of each person and his or her family.
